Call Verisma Customer Service at 866-504-5155. If you received a “Medical Records Request Received” letter from Verisma, go to https://track.verisma.com and enter your Access Code to track your request and download records (if available).

Most requests are processed within 3–10 business days after Verisma receives and validates your request. Times can vary based on the size and complexity of your request. Federal regulations allow up to 30 days to complete a request.

Please enter through the main patient entrance and inform security you are picking up medical records. The Medical Records office is on the 3rd floor in the MPA office.
No. We do not charge patients for copies of their medical records.
Only certain requesters, such as law firms, insurance companies, or those requesting records for disability or legal purposes, may be charged as allowed by law. In most cases, records are not prepaid and are sent with an invoice after processing, when applicable.
If a prepayment fee applies, our release of information partner, Verisma, will notify the requester in advance and provide payment options before the request is completed.
To protect your privacy, you may be asked to show a government‑issued photo ID or provide documents that prove legal authority (e.g., for a minor, a patient representative, or an estate).
If you think something is wrong in your medical record, call the MPA Medical Records Department
at 316-293-1842 to ask about the record amendment process.
